Caramelize the Onions
Heat olive oil in a large skillet over medium-low heat. Add the sliced onions and a pinch of salt; cook, stirring occasionally, until deep golden brown and very soft, about 12 minutes. Sprinkle with sugar halfway if desired to speed browning.
Cook the Macaroni
Meanwhile, bring a large pot of salted water to a boil. Add macaroni and cook until al dente (about 7-8 minutes). Reserve ½ cup pasta water, then drain.
Make the Mozzarella Sauce
In the same pot used for pasta, melt butter over medium heat. Whisk in flour and cook for 1 minute. Slowly pour in the milk, whisking constantly, until smooth and thickened (about 3 minutes). Remove from heat and stir in shredded mozzarella until melted and creamy. Season with salt and pepper.
Combine and Serve
Add the drained macaroni and caramelized onions to the cheese sauce. Stir gently to coat evenly, thinning with a splash of reserved pasta water if needed. Serve immediately while hot and creamy.
